*	What type puppy is your choice ,colors and sizes ..... Maltipoo, Maltepom.......if possible always put a first and second choice of type ,sex,color and size and note whether you will take 1st generation or second generation or either .Maltepoms are generally white . Colors of maltipoos...black ,black white,apricot or other ..... Sizes...5 to 10 lbs ...8 to 12 lbs ...Over 10 lbs

*	How many other pets do you have and type of pets ?
*	On the available puppy contract ,you are required to have pet netured/spayed by 6 months of age .Are you willing to do this ? All those that fill out this contract MUST once you choose your puppy fill out the Available Puppy Contract and abide by all areas of contracts .Please type yes or no .
*	I usderstand this is a binding contract between me (put name in box) and Poos-4-U. I have read and filled out all the required questions and have sent a deposit of 200.00 .This deposit is NOT refundable ,but is taken off the total amount when I choose my puppy .
